Integrated provider network for community-based long-term care
The pilot will focus on the creation of multi-disciplinary local teams working closely to the community and care users, actively collaborating and signaling care and support needs to other parties and organizations in the area.
Over the next years we will
- organize regular regional stakeholder workshops,
- work closely with existing local care management platforms,
- establish new links between local authorities, care providers and care users
- organize trainings for care professionals in the area of care management and integrated care delivery.
By doing so, we will orient ourselves at the overall national strategy for long-term care in Austria.

Integration of an Emergency Button Service within a home-care service package
The Red Cross of North Macedonia will focus on piloting an innovative community-based approach to care for dependent elderly, through the integration of an Emergency Button Service within its home care package. The pilot will target frail older people living independently in their homes but who are at risk of adverse health outcomes and whose support needs are insufficiently met by existing community-based services. 50 older persons will be recruited from the pool of beneficiaries of the RCCS. Within the home-care package, the RCNM through the caregiver trainings program will also ensure 60 caregivers receive extensive support to enhance their theoretical, psychosocial and practical skills.

Support for family caregivers of people with dementia within an integrated service approach
The pilot aims to encourage reciprocity in non-professional care relationships, with special attention to supporting the well-being of family caregivers of older people with dementia and to promote a more equitable distribution of care within households. It centers on the provision of individual psychological support and the creation of peer-to-peer support groups in order to ensure that the training, self-care and psychological needs of family caregivers of people with dementia are properly met.
